Thanks for the tip scav, I hadn't thought to try disabling my anti-virus. Luckily I can set it to ignore the game folder so I can keep it on.

Is this fix for the Steam version still working for people? When I try to start it, three copies of the .exe show up in the task manager and Steam crashes before the game can even start. For some reason, I can't even end the processes in the task manager.

I used to run a mixed configuration with DVI and D-Sub and I didn't really notice a difference.

The zip has the program .exe and a .dll file. I tried this fix and I think it might actually be working without a no-cd crack despite what the instructions say.
